An uncontained engine failure on American Airlines Flight 383, a Boeing 767, resulted in a fire at Chicago’s O’Hare International Airport. The National Transportation Safety Board (NTSB) identified a manufacturing defect in a General Electric engine component as the cause. In addition to the equipment failure, the NTSB report criticized the pilots and flight attendants for a flawed evacuation that led to serious injuries.
