Indonesian investigators concluded that the design of Boeing’s 737 Max and insufficient regulatory oversight were contributing factors to the fatal Lion Air crash that killed 189 people. The investigation found that flawed assumptions were made during the plane’s design and certification regarding pilot responses to malfunctions with the Maneuvering Characteristics Augmentation System (MCAS). The MCAS system’s reliance on a single, mis-calibrated angle of attack sensor made it vulnerable. Furthermore, a critical safety alert feature was not correctly enabled during the aircraft’s development, preventing maintenance crews and pilots from identifying the sensor problem.
